New Patient Information

Thank you for choosing Tri-County Pediatrics.  Please print out and complete the PATIENT INFORMATION SHEET, MEDICAL HISTORY FORM and FINANCIAL POLICY, below and bring them with you to your first visit.  Also please read our HIPPA PRIVACY POLICY.  If you are transferring from another pediatric practice, please print out and complete our RECORDS RELEASE FORM and submit it to your child's previous physician.  We must have your child's records at our office before we can schedule your child's first Well Child Exam.

If you subscribe to a managed care insurance policy (HMO), please notify your insurance company of your change of primary care physician.  We cannot provide medical care to your child until this change has been made.

If your child is a newborn, please make sure that they have been added to your insurance policy.  Typically, there is a limited time after your baby is born to add them to your policy without having to wait for the next open enrollment period.  Contact the administrator of the health policy (for example, the primary subscriber's Human Resource office) for more details.

If your children are covered under MassHealth, Tri-County Pediatrics accepts the PCC plan only.  Services rendered for a child under any other form of MassHealth will not be covered and will be your financial responsibility.  Call Member Service Center of MassHealth at 800-841-2900 for assistance.

What's New?
On July 1, 2019, Dr. Philip Hourigan will retire from the practice of pediatric primary care.  Dr. Hourigan has been caring for infants and children for over 35 years.  His practice consists of three generations of families.  He will be greatly missed by his patients and colleagues.  We all join in wishing him a happy and healthy retirement.
